Online Option

All students participating on a Pacelli Sports team are required to have a current athletic physical and all related medical permission forms on file.

Please complete all forms online through Healthy Roster, with the exception of the GIAA Preparticipation Physical Form, which must be printed and completed by a physician. Once the physical is completed, upload the signed form to Healthy Roster.

Your student’s physical and required paperwork must be submitted before they can participate in any workouts or tryouts.
